Your Visit

At the time of your visit, you will be asked to complete a Patient Registration Form and a Health History Form.
 We can mail them to you or they can be printed. Either way, please complete them in full and bring them with you to your appointment.

Please Check with Your Insurance Company(s) to Verify Your “Vision Care” Coverage
 Please remember that ultimately it is your responsibility to know your insurance plan and benefits. Recent trends lean toward annual benefit changes in your health plan, typically reducing the amount of coverage or type of benefits offered. You should also be aware that most insurance companies do not cover routine eye exams. If you have only medical coverage, without a medical complaint or medical diagnosis, you will be responsible for your bill.

Check with your primary care doctor, if listed on your insurance card, to determine if a referral is required for coverage.

The Day of Your Appointment

We will ask to make copies of your insurance card(s) and a photo ID. 
A photo ID and insurance cards are required if you would like for us to file your claim to your insurance company. Please make sure you bring the most current insurance cards with you, as insurance companies frequently modify benefits. If your insurance, address or phone number changes at any time throughout the course of your care with us, please notify us promptly.

The co-pay amount listed on your insurance card will be collected at the time of your visit, if applicable.You will be responsible for any co-pays and deductibles, as well as any services not covered by your insurance policy. For charges not covered by your individual insurance plan, we require payment in full at the time services are rendered. This usually includes the charge for a refraction and contact lens fittings. We accept cash, check or Visa, MasterCard, or American Express as payment for services.

Bring sunglasses with you to your appointment.
 As part of your examination, your pupils may be dilated. With dilation, most people experience light sensitivity and difficulty with close focusing (such as reading). Some individuals also have difficulty seeing in the distance. The effects of the dilation can last up to several hours. We have disposable sunglasses available for our patients. Please ask a staff member if you should have any questions or concerns about dilation.

If you cannot keep your appointment, please notify us as soon as possible.You may be charged a $25.00 fee for missed appointments or ones not cancelled at least 24 hours prior.
